How to Apply for a MAST²CARE Center?
Thank you for your interest in the MAST²CARE network!
If you would like to become a MAST²CARE member we would like to give you some information about the simple application process.
Please fill out the application form below and send it to the MAST²CARE Audit office. Upon receiving your application, we will look for auditors and coordinate an audit visit. Every certified center can audit other centers. After the audit visit, the MAST²CARE steering committee will review the audit report and inform you about the results. If the audit was successful, you will receive a certificate from us that is valid for two years. After these two years there we will coordinate a re-audit with you.
